To help your first appointment run smoothly, we ask new patients to complete a few forms before arriving at our Amarillo office. Filling these out ahead of time allows our team to prepare for your consultation and gives Dr. Chapman a clear picture of your dental history before you sit down together.
If you have any trouble accessing or completing a form, our front office team is happy to help over the phone or have a paper copy ready for you when you arrive.
